Job DescriptionAs a Clinical Nurse, you will provide advanced clinical expertise, leadership, and patient-centred care to support individuals with complex health needs. This role is pivotal in ensuring safe, high-quality outcomes and continuity of care from hospital to home.
- Delivering specialist nursing care, education, and support to patients with heart failure across hospital, clinic, home, and telehealth settings.
- Leading and coordinating care within a multidisciplinary team, ensuring best practice and adherence to clinical standards.
- Supporting the development of staff through mentoring, education, and role modelling.
- Contributing to quality improvement, research, and innovation to enhance service delivery and patient outcomes.
To succeed in this role, you must have:
- Demonstrated expertise in cardiac/heart failure nursing, including assessment, care planning, and evaluation in complex clinical situations.
