Occupational Therapist in Community Care

We are seeking an experienced Occupational Therapist to join our community-based team. As a key member of our multidisciplinary team, you will work closely with clients and their families to develop personalized care plans that enhance their independence and quality of life.
The ideal candidate will have a strong understanding of evidence-based clinical practice and the ability to provide targeted occupational therapy services to clients with complex health conditions or age-related care needs.
Responsibilities- Provide high-quality, client-centered occupational therapy services in the community setting.
- Collaborate with other healthcare professionals to ensure seamless care coordination and optimal client outcomes.
- Prescribe and implement appropriate aids, equipment, and home modifications to support client goals.
- Degree in Occupational Therapy with current AHPRA registration.
- Minimum 2 years' experience in community-based occupational therapy, preferably with a focus on Home Care Package and Commonwealth Home Support Program.
- Valid driver's licence and insured vehicle for travel to client homes.
