Job Title: Dedicated Community Nurse

We are seeking a compassionate and skilled nurse to join our team and help improve the lives of older adults living independently in the community. The successful candidate will provide high-quality care to elderly clients, drawing on primary health care principles to deliver holistic, person-centred care.

About the Role

This is a meaningful opportunity to make a real difference in the lives of older people—helping them stay well, stay safe, and stay connected.

  • Providing direct clinical nursing care to clients in their homes, working closely with multidisciplinary teams to ensure holistic healthcare and wellbeing.
  • Conducting clinical assessments, providing safe, quality nursing practice to clients with specific needs and/or complex needs.
  • Liaising with all parties involved in the care of the client to maintain accurate documentation.
Requirements

To be successful in this role you'll need:

  • A genuine passion for providing high-quality care to the elderly.
  • Current AHPRA registration following completion of a Bachelor of Nursing or similar.
  • Minimum 3 years post-graduate nursing experience.
  • Experience in clinical assessments and initiative to work independently.
