Case Manager Role OverviewWe are seeking a highly skilled Case Manager to join our team and make a real difference in the lives of older Australians.The successful candidate will be responsible for overseeing the day-to-day care for clients, providing direct support and ensuring all care is delivered to the highest standard by skilled staff members.In this role, you will work closely with a supportive team to ensure clients receive the best possible care. You will also contribute to initiatives that positively impact people's lives and help grow our client base.
  • Key Responsibilities:
    • Deliver high-quality care and support to clients
    • Oversee daily operations and manage client files
    • Collaborate with the Senior Care Manager to achieve regional goals
    • Bid on new business opportunities to grow the client base

Requirements:
  • Experience in aged care and client coordination

  • Commitment to meeting Aged Care Standards and legislative requirements
  • EEN or equivalent qualification
  • Current First Aid certificate and National Police Clearance
  • Current Driver's Licence and fully maintained vehicle
