A professional in patient-centered care is sought to join our team within the Transitional Aged Care Program at NSW Health.

Key Responsibilities:
  • Deliver high-quality services that support patients and their families through slow-stream therapy.
  • Conduct home visits, provide guidance to caregivers, and contribute to patient-centred care.
  • Participate in ongoing training, clinical supervision, and peer support.
About the Role

You will be responsible for delivering social work services as part of a dedicated team supporting patients and families.

Requirements
  • Bachelor degree in social work or equivalent qualification.
  • Unrestricted NSW Driver's Licence with ability to visit clients at home across the Lower Mid North Coast of NSW.
