HCP Care Coordinator Specialist
About the Role:We are seeking a skilled HCP Care Coordinator Specialist to join our team. As a key member of our organization, you will be responsible for participating in continuous quality improvement activities, conducting post-discharge follow-up to assess patient progress, addressing concerns, and preventing readmissions.
Key Responsibilities:- Analyze data to identify trends, areas for improvement, and opportunities for optimization
- Manage the referral process
- Evaluate and enhance client care plans
- Maintain accurate and up-to-date client records
- Bachelor's degree in Nursing or relevant field
- Experience as an HCP Case Manager
- Strong organizational, communication, and interpersonal skills
- Knowledge of home care regulations and industry best practices
- CPR and First Aid certification
