Physiotherapist Role

We are seeking a Physiotherapist to join our Home and Community Care team in the South Metropolitan area of Perth.

The successful candidate will use evidence-based interventions and a re-ablement approach to improve the independence of customers in an autonomous role that requires travel in the South Metropolitan area providing care to our customers in the community.

Key Responsibilities:
  • Conduct comprehensive assessments, develop personalized care plans, and implement effective strategies to meet each client's unique needs.
  • Design both individual and group exercise programs to keep clients motivated and engaged to optimize their function and independence.
  • Through equipment assessment and prescription, identify and provide solutions that support client autonomy and well-being.
  • Foster collaboration and successful outcomes by communicating effectively with clients, families, and caregivers.
  • Ensure seamless care coordination and accurate progress tracking through meticulous record-keeping.
  • Work closely and collaboratively with the multidisciplinary team and Therapy Assistants to achieve the desired outcomes for your clients.
Required Skills and Qualifications:
  • Current FULL registration with AHPRA as a Physiotherapist is essential.
  • Ability to relate to elderly customers with empathy whilst working effectively with other health professionals and service providers.
  • Excellent interpersonal and communication skills.
  • Proven ability to assess, plan, implement, and evaluate quality care.
  • Current Australian driver's licence and reliable vehicle are both essential.
