At Anglicare Southern Queensland, we are seeking an experienced Team Leader to join our Home and Community Service in Toowoomba. The ideal candidate will have a proven track record in leading high-performing teams and delivering exceptional customer service.

The successful applicant will be responsible for supporting the delivery of community services to clients, facilitating individualised care plans, and supervising a team of Home Care Workers and Support Services Workers. They will also be required to assist with orientation of new staff, provide guidance and support, and ensure that all client-related documentation is up-to-date and accurate.

Responsibilities
  • Supervise Home Care Workers and Support Services Workers on a day-to-day basis.
  • Assist with orientation of new staff and provide guidance and support.
  • Facilitate and monitor individualised care plans to meet client needs.
  • Support the delivery of community services to assigned clients.
Qualifications and Experience
  • Previous experience in a senior care worker or supervisory role.
  • Able to lead, manage, train and mentor team members.
  • Possess highly developed interpersonal skills.
  • Demonstrated time management skills.
  • Current National Police Certificate.
  • Current Driver's Licence and own reliable vehicle.
  • Certificate IV in Home and Community Care or equivalent.
  • Current First Aid and CPR Certification.
