Job Title: Dedicated Support CoordinatorAbout the Role

We are seeking a highly skilled and compassionate Support Coordinator to join our team. As a key member of our organization, you will play a vital role in coordinating individualized support and services for our clients living with dementia.

Key Responsibilities:
  • Monitoring service delivery to ensure alignment with client goals and objectives;
  • Maintaining accurate records and data on staff, clients, and carers;
  • Identifying gaps in service provision and implementing continuous improvement initiatives;
  • Collaborating with colleagues to develop and implement team procedures;
  • Coordinating referrals to ensure access to appropriate services for individuals with dementia;
  • Providing dementia-specific information and education to clients and carers;
  • Facilitating communication between staff, clients, and external agencies;
  • Supporting staff development through training, supervision, and performance evaluations;
  • Contributing to the recruitment and onboarding process as required;
  • Ensuring safe working conditions for community and respite staff;
  • Liaising with internal and external stakeholders to promote best practice and service excellence.

Requirements:

  • Degree in a relevant field or significant experience in a related role;
  • Excellent communication and interpersonal skills;
  • Ability to work autonomously and as part of a team;
  • Strong organizational and time management skills;
  • Commitment to delivering high-quality services that prioritize client needs and dignity;
  • Familiarity with dementia care principles and practices.
