War Memorial Hospital TACP Registered Nurse PFT Position

War Memorial Hospital (WMH) is a leading provider of aged care and rehabilitation services in the community. We are seeking a highly skilled and experienced Registered Nurse to join our Transitional Aged Care (TAC) team.
About the RoleThis is a permanent full-time position, working 76 hours per fortnight, with flexible start and finish times. The successful candidate will be responsible for planning, coordinating, and delivering high-quality nursing interventions to patients at WMH, consistent with AHPRA, Uniting, NSW Health, and South Eastern Sydney Local Health District (SESLHD) policies, procedures, and standards.
Key Responsibilities- Provide comprehensive, client-centered nursing assessments, care, and case management of older people, particularly those with dementia or complex health care needs.
- Work effectively and autonomously as part of a multi-disciplinary team, exercising independent professional judgment on routine matters.
- Delegating and supervising nursing duties to other nursing/care personnel, ensuring quality, effective clinical care standards are maintained with efficient use of resources.
- Participate in quality initiatives within the TAC team and/or hospital, demonstrating a commitment to quality improvement, evidence-based practice, and ongoing clinical and professional learning.
- Minimum 2 years' experience as a Registered Nurse in the area of aged care, rehabilitation, or other relevant specialty, with postgraduate qualification or interest in working towards same.
- Demonstrated experience or interest in working in the community, including ability to perform comprehensive, client-centered nursing assessments, care, and case management of older people.
- Current registration as a Registered Nurse with the Nursing & Midwifery Board of Australia and current NSW Drivers' license Class C.
