Job Title: Community Support Specialist

Role Overview:

We are seeking a skilled and compassionate Community Support Specialist to provide essential assistance and support to individuals with physical and mental health conditions.

Main Responsibilities:

  • Provide personalized care and support to clients, including assistance with daily living activities.
  • Develop and implement tailored care plans in collaboration with clients and their families.
  • Support clients in accessing community resources and services.
  • Maintain accurate records of client progress and report any changes in their condition.
  • Foster a supportive and respectful environment for clients, ensuring their dignity and rights are upheld.

Essential Qualifications:

  • Education: Minimum of a Certificate III in Individual Support or equivalent qualification.
  • Experience: Previous experience in a similar role is preferred but not mandatory.
  • Key Skills:
    • Strong communication and interpersonal skills.
    • Ability to work independently and as part of a team.
    • Empathy and patience in dealing with clients.
    • Basic computer skills for documentation and reporting.
