We are seeking an experienced Support Specialist to join our team at Mission Australia, working with individuals who have experienced severe and recurring mental illness. As a Support Specialist, you will play a critical role in providing ongoing case management and practical supports to clients, aiming to improve mental health outcomes and enhance overall wellbeing.
Key Responsibilities- Respond to referrals of clients to the service from internal and external support services.
- Provide outreach services to clients, successfully inducting them into the program and creating individualized support plans.
- Develop and maintain relationships with internal and external stakeholders, including service partners and government agencies.
- Administration - Complete a range of internal and external reports relating to clients and the program, including risk assessments, individualized support plans, and program promotion.
- Relevant experience or qualifications in community services.
- Demonstrated experience of case management working with people with complex mental health issues.
- Skills in building rapport with clients while also liaising, negotiating, and advocating for consumers with a wide variety of stakeholders.
- Computer literacy, preferably using Windows.
- First Aid certificate and Driver License.
- Willingness to undertake any relevant training opportunities for professional development relevant to the role.
