Position Overview:
We're seeking experienced enthusiastic, compassionate, and highly organised Support Coordinators to join our vibrant team. In this role, you will be the essential link between participants and the services they need, helping them navigate their NDIS plans to unlock their full potential. You'll also gain valuable skills that will prepare you for a career in navigation services, helping people find their way through the service system.
Key Responsibilities:
- Establish strong relationships with participants and their families to create a connected life outside of funded support.
- Provide expert guidance to participants as they implement their NDIS plans, ensuring they have the necessary support to make informed decisions.
- Coordinate the implementation of supports, ensuring capacity and funding management are optimised for each participant.
- Support participants in collecting key evidence to support their changing needs, and facilitating funding requests when required.
- Explore opportunities in recreation, employment, housing, and more to enrich the lives of our participants.
- Ensure mainstream services step up and fulfill their commitments, promoting a seamless support experience.
- Provide regular, insightful reports to the NDIA and participants on progress and achievements.
- Achieve all relevant KPIs, including billable hours, to ensure high-quality support delivery.
Qualifications & Skills:
- Qualifications in social work, disability, mental health, or a related field.
- Experience in NDIS support coordination, preferably with a strong track record of delivering high-quality results.
- In-depth knowledge of the NDIS and its range of services, with a keen understanding of how to leverage these to best support participants.
- Confident in meeting KPIs, including billable hours, with a proven ability to adapt to changing circumstances.
- Excellent communication skills – you'll be dealing with diverse participants, providers, and stakeholders.
- A natural problem-solver, multi-tasker, and decision-maker with a keen attention to detail.
- Eligible for or possess a Blue Card and NDIS Worker Screening Clearance.
