As a Clinical Care Coordinator, you will work closely with our multi-disciplinary team to ensure efficient and high-quality service delivery. Your primary responsibility will be to coordinate the clinical team, working in collaboration with the Nursing Regional Manager and administration team.
Key Responsibilities- Provide clinical management and coordination for our clients and partnering residential aged care facilities.
- Work autonomously and appreciate the freedom and flexibility in this role.
- Regular travel to partnering homes to facilitate clinical review.
- Support the model by conducting business development to grow the resident base.
- Proficiency in computer usage and ability to document customer needs.
- Bachelor's or Diploma of Nursing.
- Experience in problem-solving and professional collaboration.
- IT skills and proficiency in documenting customer needs.
- AHPRA Nursing Registration.
- Aged Care experience, leadership preferred.
- Current driver's license and reliable vehicle.
