Health and Wellbeing Coordinator Position

This is an exciting opportunity to join our team as a Health and Wellbeing Coordinator, responsible for supporting the health and wellbeing of our residents within our new Supported Independent Living services.

We are seeking a qualified individual with experience in developing and implementing holistic health and wellbeing programs for residents in supported independent living facilities. The successful candidate will have excellent interpersonal and communication skills, with the ability to build strong relationships with residents, families, and multidisciplinary teams.

Key Responsibilities:

  • Developing, implementing, and evaluating holistic health and wellbeing programs for residents in our supported independent living facilities
  • Collaborating with multidisciplinary teams to ensure comprehensive care and support for residents
  • Monitoring and reporting on resident's health and wellbeing, and adapting programs as needed
  • Organising and facilitating social, recreational, and therapeutic activities to promote active and engaged lifestyles
  • Providing guidance and support to residents and their families to enhance their health, independence, and quality of life
  • Maintaining accurate records and documentation in line with regulatory requirements

Required Skills and Qualifications:

  • Relevant tertiary qualification in an allied health, nursing, or community services discipline
  • Extensive experience in a similar role within the aged care or disability support sector
  • Exceptional interpersonal and communication skills with the ability to build strong relationships with residents, families, and multidisciplinary teams
  • Demonstrated expertise in developing and implementing health and wellbeing programs that promote independence and quality of life
  • Strong organisational and problem-solving skills with the ability to prioritise and manage competing demands
  • Commitment to providing person-centred care and upholding the rights and dignity of vulnerable individuals
