Rural Generalist Social Worker

We are seeking a highly skilled and experienced social worker to provide comprehensive services in our rural healthcare settings.

About the Role:
  • Deliver high-quality social work services across diverse healthcare environments.
  • Collaborate with a multi-professional team to support consumers through their patient journey.
  • Apply clinical expertise to deliver wellness, re-ablement, and empowerment services tailored to the local rural context.
Key Responsibilities:
  • Support consumers through the patient journey, providing effective assessment, timely referrals, accurate information, co-ordinated care, and prompt follow-up.
  • Apply clinical skills to a broad scope of practice, delivering services that promote wellness, re-ablement, and empowerment appropriate to the local rural context.
  • Contribute professional leadership in the application of clinical protocols and standards within the multi-disciplinary team and health unit/LCLHN.

Requirements:
  • A recognised qualification within the social work profession.
