Job Title: Clinical Nurse Specialist - Endoscopy and Chronic Disease
Reports To: Nursing Management Team and Practice Manager
Direct Reports: None
Role Purpose: To provide high-quality, safe, and efficient nursing services to patients and healthcare practitioners within the medical centre.
Principal Accountabilities:
- To carry out activities to ensure that patients and healthcare practitioners are provided with high-quality, safe, and efficient nursing services within the medical centre.
Key Responsibilities:
- Financial Management: To manage financial resources effectively and efficiently.
- Reaching Set Targets: To achieve set targets and goals in a timely manner.
- Meeting KPS's (If Applicable): To meet Key Performance Standards (KPS) as applicable.
- Operational Effectiveness: To ensure that clinical areas are adequately stocked and take action to replenish stock before depletion ensuring stock is in date.
- Direct Patient Care Activities: To conduct a range of direct patient care activities in the treatment room and in accordance with company Standard Operating Procedures (SOPs) and/or Standing Orders.
- Chronic Disease Management: To assist the GP with conducting health assessments, GP management plans, team care arrangements, reviews, and initiate other chronic disease management tasks in accordance with Medicare requirements.
- Nurse Led Clinics: To collect health history, plan, coordinate, and document patient care, and evaluate patient outcomes related to patient condition(s) and/or presentations in accordance with ForHealth endorsed policies and standing orders.
- Clinical Administration: To ensure clinical areas are tidy, clean, and presentable, and to conduct routine activities as defined in the Treatment Room complete required checklists.

Minimum Requirements:
- Qualifications: Current AHPRA registration as a Registered Nurse (RN) and Bachelor's degree in nursing.
- Experience: 2 years' experience providing direct clinical nursing care desirable.
- Critical Skills: Understanding of health assessments, chronic diseases, prevention principles, and health promotion, and basic computer skills.
