About the RoleThe successful candidate will be responsible for providing direct clinical services to patients and their families within the assessment and treatment framework of social work knowledge and skills. This includes assessing the psychosocial needs of patients and aiding them in connecting with programs in the community to meet these needs.
Responsibilities- Provide consultation to physicians, nurses, and other relevant disciplines concerning the social and emotional factors that bear upon the patient's treatment.
- Maintain close communication with the physician and other professionals directly involved with the patient's treatment.
- Plan and coordinate, where appropriate, programs in areas of social work expertise.
- Act as an advocate on behalf of patients and their families, both in hospital and in the community.
- A Master's Degree in Social Work from an accredited School of Social Work.
- A member of the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum 3 years previous related experience preferred.
