Job Description

The role of a Support Coordinator is pivotal in ensuring seamless coordination of support services for clients and assisting them in achieving their goals and aspirations.

We are seeking an experienced professional to join our team as a Support Coordinator, providing exceptional service to our clients and contributing to their success.

Key Responsibilities
  • Engage with service providers to coordinate support services and monitor progress toward plan goals.
  • Assist customers in identifying community or mainstream supports to achieve set plan goals.
  • Develop and support programs promoting skill development and community involvement.
  • Promote connections and networking opportunities outside the service system, aligned with individual goals.
  • Help customers understand and implement their plan.
  • Establish links with NDIS, TLC programs, and other provider services.
Requirements
  • Ability to engage with the community and create meaningful relationships.
  • Experience applying best practice, maintaining integrity around legislation and NDIS guidelines.
  • Coordinating complex client caseloads.
  • Developing programs promoting skill development and community involvement.
  • Collaboration with NDIS and related services.
