Integrated Care Coordinator

About the Role:

This position plays a key role in our multidisciplinary team delivering high-quality mental health services. The Integrated Hub works closely with peer-led services, including the Safe Haven and the Recovery College.

Main Responsibilities:
  • Oversight of service optimization for attendees
  • Supervision and support of Peer Workers within the Integration Hub
  • Leadership, direction, and change management to support service development and compliance
Key Responsibilities:
  • Contribute to the culture, leadership, and daily management of the Integration Hub
  • Support and educate Peer Workers, providing regular debriefing opportunities
  • Support consumers and their families/friends as leaders of their own care
