Role Overview

We are seeking a compassionate and skilled Social Support Services Worker to join our team at Castlemaine Health. This rewarding role involves providing essential services to frail, aged, or permanently disabled individuals and their carers.

About the Role

This position requires a Certificate III in Aged Care or Disability Services, demonstrated experience in supporting aged and frail or disabled individuals, ability to work both independently and collaboratively within a team, and proficiency in providing recreational and support services in a group setting.

Responsibilities
  • Activity Planning: Develop and implement diverse activity programs tailored to meet the needs and interests of our participants, fostering a supportive community environment.
  • Advocacy and Support: Provide advocacy, support, and information to empower individuals and their families to access relevant services and resources, promoting independence and connection.
  • Collaboration: Work collaboratively with team members and external stakeholders to ensure holistic and coordinated care for our participants, upholding Dhelkaya Health's values of integrity, compassion, and respect.
  • Documentation: Maintain accurate records and documentation in accordance with organizational policies and funding requirements.
Key Requirements
  • A valid driver's license is an essential requirement for this role.
  • Experience undertaking group activity programs, especially with older adults and individuals with disabilities.
  • Strong consultation, planning, and facilitation skills to create meaningful and engaging activity programs.
  • Excellent communication skills, both verbal and written, and the ability to work collaboratively within a team.
