Maximizing Client Potential through Effective Care Planning.

Our organization is seeking an experienced professional to support and expand our home care services in a designated region. Working closely with a regional team, the position will be responsible for developing effective relationships to grow the home care services program locally and managing a caseload of clients.

Key Responsibilities:

  • Conducting client consultations to understand their needs and develop tailored care plans.
  • Designing and maintaining goal-oriented care plans to ensure clients receive optimal care.
  • Monitoring and reviewing client services, coordinating support services to meet individual needs.
  • Managing client expenditure and ensuring budgetary efficiency.

Requirements:

  • A genuine passion for helping individuals within the community, accompanied by strong relationship-building skills.
  • Previous case management experience and relevant qualifications.
  • Current Victorian Driver's Licence.
  • Excellent communication and organizational skills.
  • A practical understanding of physical limitations and ability to adapt to individual needs.
  • Flexibility to respond to changing client needs.

To succeed in this role, you must demonstrate or be obtaining National Disability Insurance Scheme (NDIS) Clearance and First Aid Certificate with CPR, as well as proof of current influenza vaccination.
