Job Overview

Senior healthcare specialists play a vital role in ensuring the well-being of older adults by assessing their needs and connecting them with necessary services that support independent living.

Main Responsibilities
  • Conduct thorough assessments to identify the needs of older adults, including those related to healthcare, transportation, and home maintenance.
  • Facilitate referrals to services that promote independence, such as healthcare, transportation, and home care.
  • Work closely with families and carers, recognizing them as key stakeholders in the care process.
  • E nsure all assessments are conducted in line with relevant contractual requirements and regulatory standards.
Required Skills and Qualifications
  • At least 1 year of experience in healthcare, including client assessments and/or service delivery.
  • Tertiary education in a health-related discipline, such as nursing, occupational therapy, physiotherapy, or social work.
  • Current unrestricted general registration with a relevant professional association.
  • Proficiency in using software applications, including Microsoft Office and client management systems.
  • A current driver's license and comprehensively insured vehicle.
