About the Role

We are seeking a dedicated Care Coordination Specialist to join our team in Townsville. In this vital role, you will work closely with clients, families, and healthcare professionals to ensure smooth transitions and the delivery of high-quality care.

Key responsibilities include:

  • Leading and collaborating with interdisciplinary teams to implement program objectives
  • Providing guidance and supervision to program staff to ensure the highest standards of care and service delivery
  • Developing and implementing comprehensive therapeutic care plans for clients, considering their unique needs and circumstances
  • Conducting consultations with community services to enhance collaboration and support networks for clients
  • Maintaining accurate and up-to-date documentation and clinical records in accordance with regulatory standards
  • Facilitating smooth and well-coordinated discharge planning in collaboration with healthcare professionals, ensuring continuity of care for clients post-transition
  • Providing support to clients and their families to address the emotional aspects of the transition period
  • Delivering education sessions to clients and families, promoting health literacy and empowering them to actively participate in their care
  • Offering ongoing support services to enhance the overall well-being of clients throughout the transition period
About You

To be considered for this rewarding role, you will have:

  • Excellent communication and interpersonal skills
  • Ability to work collaboratively with diverse stakeholders to achieve positive outcomes
  • Previous experience in program leadership and case management
  • Strong understanding of therapeutic care planning and discharge planning processes
  • Ability to work collaboratively in a multidisciplinary team
  • Effective prioritisation and time management skills
  • Excellent written and verbal communication skills
Role Requirements
  • Travel to selectability sites will be required
  • AHPRA Registration
  • A current minimum P1 'C' class Queensland drivers licence
  • A currently registered, roadworthy, and fully comprehensive insured vehicle
  • A Positive Notice Working with Children (blue card) or the ability to obtain prior to commencement
  • A current yellow card exemption or an NDIS Worker Screening Clearance or the ability to obtain prior to commencement
  • NDIS Worker Orientation Module 'Quality, Safety & You' completion certificate
