About the RoleWe are seeking a skilled Child Health Nurse to join our Jajumbora Midwifery Hub team. As a Child Health Nurse, you will have the opportunity to work in an evidence-based model that supports families in a continuity of care birthing model. You will contribute to the overall care and wellbeing of clients, helping to improve their health outcomes.
Key Responsibilities- Provide high-quality nursing practice within the Jajumbora Model of Care, working collaboratively with the team to achieve results.
- Develop and implement effective care plans that meet the unique needs of clients.
- Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
- Participate in quality improvement initiatives to enhance patient outcomes and experiences.
- Work collaboratively with multidisciplinary teams to achieve better health outcomes for clients.

Requirements- Bachelor's degree in nursing or higher.
- Full un-restricted AHPRA registration.
- Immunisation Endorsement and experience.
- Previous demonstrable experience in relevant maternal and child health nursing field.
- Demonstrated ability to meet targets and performance outcomes.
- Ability to support a continuous improvement model for achieving outcomes.
- Decisive listening, influencing, and persuasive skills.
- Working with Children's Card (Blue Card) or be eligible to apply.
- QLD Yellow Card.
- Previous experience of working with Aboriginal and Torres Strait Islander clients.
- Previous experience in a multidisciplinary health team.
- Previous experience in chronic disease management.
