About the Job:

Job Summary:

  • The Senior Social Worker will play a key role in implementing outcome measures to evaluate therapy programs in collaboration with an interdisciplinary team.
  • Responsibilities include independently assessing clients' abilities, problems, and needs using formal and informal assessments as needed.
  • Care planning, case management, and liaison services will be provided to ensure sustainable social support in place in the community.
About You:

Requirements:

  • A critical thinking ability to analyze issues related to clinical care in social work for quality activities, service evaluation, or research.
  • Experience in providing care planning, liaison, and case management services.
  • Extensive experience in aged care, with high levels of responsibility, initiative, and organizational skills.
