We are seeking a dedicated General Practitioner to join our Aged Care GP Program in the NSW region. As a key member of our team, you will play a vital role in improving access to healthcare for residents in aged care facilities.

Our Collaborative Care Model brings together a team of doctors, nurses, geriatricians, carers, and allied health professionals to support the healthcare needs of residents.

Job Overview

This is a part-time opportunity, with 16 hours per week, working on a refer and request basis. You will be required to visit or conduct telehealth consultations with residents in the Dubbo and Condobolin areas. The ideal candidate will be based in Dubbo or Condobolin, with occasional travel to other facilities.

Key Responsibilities
  • Conduct regular consultations with residents to monitor their health and wellbeing
  • Develop and implement individualized care plans in collaboration with the care team
  • Provide education and support to residents and their families on healthy living and disease management
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
Requirements
  • Vocational or General Registration with the Australian Health Practitioner Regulation Agency
  • Current full Australian work rights
  • Experience in Emergency Department and/or General Practice
  • Ability to work independently and as part of a team

Estimated Salary
