We are seeking a highly skilled and dedicated Support Coordinator to join our team at uWorkin. As a Support Coordinator, you will play a vital role in assisting NDIS participants in implementing their plans, increasing their capacity, and connecting with their community.
Key Responsibilities:
- Action referrals promptly and efficiently.
- Serve as the main point of contact for service issues, complaints, and plan reviews.
- Provide participants with information to make informed decisions about services, ensuring they have a clear understanding of their options.
- Assist in engaging services and negotiating service details and costs, ensuring the best possible outcomes for participants.
- Link participants to mainstream or community services, if needed, to support their overall well-being.
- Liaise with plan managers for correct claim categories and funding, ensuring participants receive the support they need.
- Arrange assessments for funding needs, working closely with participants and service providers to ensure a smooth process.
- Strengthen participants' capacity to coordinate and manage supports, empowering them to take control of their lives.
- Assist participants in understanding service agreement responsibilities, ensuring they are aware of their obligations and expectations.
- Prepare participants for plan reviews and collect necessary reports, ensuring a seamless process.
- Network to help individuals achieve their personal outcomes, building strong relationships with participants, families, carers, and community members.
- Manage conflicts of interest per NDIA terms, upholding the highest standards of professionalism and integrity.
- Report on goals and outcomes within agreed timeframes, providing regular updates to stakeholders.
- Enhance uWorkin's reputation through continuous improvement and innovation, staying ahead of the curve in the disability services sector.
- Develop relationships with participants, families, carers, NDIA, service providers, and community members, building a strong network of support.

- Budget management, service agreements, and record maintenance experience, able to manage finances and maintain accurate records.
- Knowledge of person-centered planning and active support, able to develop and implement personalized plans.
- Commitment to Disability Services Act, WHS, and Equal Opportunity standards, upholding the highest standards of professionalism and integrity.
- Proven knowledge of NDIS Pathway, plan breakdowns, funding, reviews, processes, referrals, and report writing, ensuring compliance and best practice.
- Excellent written and verbal communication skills adaptable to participants' needs, able to communicate effectively with diverse stakeholders.
- Excellent time management and caseload prioritization skills, able to manage complex caseloads and prioritize tasks.
Mandatory Requirements:
- First Aid and CPR - A current certificate in First Aid and CPR, or the ability to obtain before starting.
- Current Drivers Licence.
- Police Check.
- Working with Children Check (Blue Card) - A valid working with children check or the ability to obtain prior to commencement.
- NDIS worker screening check or Yellow Card.
- Right to work within Australia.
Qualifications:
- Minimum Diploma in a relevant field of study, such as disability services, social work, or a related field.
Professional Experience:
- Relevant Experience in the human services area, preferably in disability services.
- Experience in working with complex clients and families, able to develop and implement effective strategies.
- Considerable relevant experience in case management within disability services, able to manage complex caseloads and prioritize tasks.
- Experience in conducting needs-based assessments, able to identify and address participant needs.
- Experience in multidisciplinary case management across settings/domains, able to work collaboratively with others to achieve common goals.
Job Types: Full-time, Part-time, Permanent
Pay: SCHADs Award, From $33.00 per hour (Pay rate to be negotiated)
Schedule: Monday to Friday
