The Practice offers a wide range of general health services, including mental health, medicals, and chronic disease management. Our team is comprised of 6 doctors, 3 nurses, and 5 administrative staff, providing a supportive and collaborative work environment.
Job Responsibilities
- Conduct patient assessments and diagnoses
- Prescribe medications, order therapies, and recommend lifestyle modifications to manage chronic conditions effectively
- Order and analyze pathology and radiological testing
- Treat and educate patients with chronic diseases
- Develop and implement individual care plans
- Provide patient education and health literacy promotion
- Manage chronic disease cases
- Utilize software, including Best Practice
Requirements
- AHPRA Registration
- NPCCWCC
- Full Immunizations
