Aged Care Transition Coordinator

We are seeking a skilled Placement Specialist to support people in later life by coordinating safe and confident transitions from hospital into aged care settings.
- Assist with discharge planning by communicating with Aged Care providers.
- Help patients and families complete necessary documentation.
- Coordinate practical arrangements such as transportation or emergency financial assistance.
Working closely with Social Workers and Clinicians ensures care needs are understood and matched appropriately, leading to smooth and respectful transitions. Every action focuses on building trust, ensuring clarity, and creating moments of dignity in what can be a daunting process.
