Job Summary:

We are seeking a dedicated Support Coordinator to provide individualized support to individuals with disabilities. This role is ideal for someone who is passionate about helping others and values the importance of independence.

About the Role:

The Support Coordinator will work closely with our team to provide 1:1 support to our clients in their homes, assisting them with daily tasks such as personal care, meal preparation, household duties, and community participation.

Key Responsibilities:

  • Assist clients with daily living tasks
  • Accompany clients to medical appointments
  • Provide emotional support and companionship
  • Develop and implement individualized support plans

Requirements:

  • Certificate 3 in Disability Support Services or equivalent
  • Minimum 6 months experience working with individuals with disabilities
  • Current Working with Children Check or in process of obtaining one
  • Valid driver's license and reliable transportation
  • National Police Certificate and NDIS Worker Screening Check
  • First Aid Certification and CPR training
