Key Responsibilities- Provide video-based telehealth services to patients with chronic conditions such as Diabetes, Chronic Heart Disease, Osteoarthritis, Osteoporosis, and Chronic Obstructive Pulmonary Disease.
- Work collaboratively with our virtually enabled remote CDM nursing team to deliver comprehensive care.
