We are seeking a skilled professional to fill the role of Clinical Case Manager. This exciting opportunity allows the successful applicant to work in a dynamic environment, contributing to the provision of high-quality care services.

Key Responsibilities:
  • Ensure client health and well-being, capabilities, independence, choice, privacy, dignity, and safety are maintained and enhanced through quality clinical care plan development and review
  • Develop and implement effective care plans for new clients, including those from Complex Care Pathway
  • Support individuals to formulate goals and aspirations within the Peace of Mind Framework
  • Maintain current knowledge and best practice in clinical care and lifestyle service delivery
Requirements:
  • Registered Nurse registration with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Manual handling certificate
  • First Aid certificate
  • NDIS Workers Check
  • Working with Children's Check
  • Clinical expertise in the management of medical conditions related to disability care
  • Experience working in disability care
  • Excellent administrative, time management, and organisational skills
  • Demonstrated positive and flexible attitude with a willingness to take on a wide range of tasks and adjust to competing priorities
  • Excellent interpersonal and communication skills with the ability to work with individuals from all socio-economic and cultural backgrounds
  • Sound ability to use the Microsoft Office suite of products
