About This Opportunity

We are seeking a highly skilled Clinical Care Coordinator to join our dynamic team. In this key leadership role, you will play a critical part in ensuring the highest quality of clinical care is provided to our residents.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to lead and motivate a team of dedicated professionals. You will also possess strong clinical expertise and a commitment to delivering best-in-class care practices.

Key Responsibilities:
  • Provision of clinical expertise and leadership of clinical and care service teams across the facility.
  • Assist in leading carer teams to meet all clinical needs of residents - including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
  • Responsible for managing all aspects of assessment, documentation, and submission of AN-ACC requirements within the facility.
  • Support the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
  • Coordination of assessment, care planning, and review processes within the facility
  • Support the admission of new residents to facilities for optimal care assessments and initial consultations with resident and families.
  • Ensure accurate information is recorded and in accordance with all organisational and legislative requirements (AN-ACC, resident records and staff time recording)
  • Input and oversight into the facility team rosters and ensuring optimal skill mix available for delivery of care to residents.
  • Manage and document clinical risk within the facility and ensure a commitment to continuous improvements within the facility.
Requirements

To be successful in this role, you will require:

  • An innate empathy for our residents and a commitment to continually improving the quality of clinical care provided to them.
  • A current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA)
  • Post graduate qualifications in aged care would be advantageous but not essential.
  • Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC and accreditation compliance and documentation.
  • An engaging style that has the ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
  • A high level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
  • A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes.
  • The ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations that arise.
