Care Team Leader Opportunity

As a Care Team Leader, you will be responsible for overseeing the provision and coordination of resident care, as well as supervising care staff to ensure an effective and efficient team environment.

• You will lead and participate in the delivery of high standard person-centered resident care.
• Coordinate comprehensive individual holistic assessments and care plans for new and existing residents.
• Ensure timely and accurate completion of clinical documentation.

Key Responsibilities:

• Lead the team in delivering a high standard of ongoing care management and perform nursing interventions.
• Assist in monitoring medication management by reviewing medication charts and storage.
• Provide direction and expert advice in the treatment of acute care episodes.

Requirements:

• Current AHPRA registration as a Registered Nurse.
• Knowledge and relevant experience in clinical services in complex nursing and/or aged care settings.
• Understanding of Commonwealth Funding System attributes related to ACFI.
• Ability to provide expert advice in treating acute care episodes.
