About the Role

This full-time position as a Care Advisor (Case Manager) supports our Bendigo Home Care Program. You will be responsible for managing a caseload of clients, advocating for them to remain living independently in their own homes.

You will facilitate discussions and develop personalised care plans that enable our clients to live independently and safely, supported by their Home Care Package.

Key Responsibilities
  1. Conduct regular in-home visits to assess and support clients
  2. Perform initial assessments of prospective clients, identifying core needs and making recommendations/referrals on capacity
  3. Develop, implement, evaluate, and regularly review client-centered care plans
  4. Work closely with Care Organisers to coordinate the delivery of services as outlined in the care plan
  5. Liaise with referring healthcare providers, allied health professionals, social workers, and community services
  6. Oversee the financial aspects of the care planning, ensuring that services are delivered in accordance with legislation and within allocated budget while maintaining high standards of care
  7. Facilitate effective client pathway support through case discussions, case conferences, and feedback reviews, including incident management and complaint resolution
  8. Maintain accurate and up-to-date records of client assessments, care plans, services provided, and any changes or incidents
  9. Contribute to teamwork and continuous improvement

This role is ideal for you if you have:

  • A clinical background as a Social Worker, EN, RN, Allied Health Practitioner, or Case Manager (highly desirable)
  • Experience managing caseloads of Home Care Package clients with complex needs
  • Expertise in safety practices within the Aged Care or Health Sectors
  • Proficiency in conducting assessments, care planning, implementation, evaluation, and documentation of quality client care in line with Consumer Directed Care principles
  • Strong communication skills to liaise with staff, consumers, service providers, and health professionals, ensuring the delivery of consumer-focused services
  • A thorough understanding of and ability to support compliance with Safety, Equal Opportunity, and other professional and legal requirements impacting client care

  • A current Working With Children Check (or willingness to obtain one)
  • Ability to pass a national police clearance
