About the Role

We are seeking an Aftercare Clinician to provide support to clients in a residential treatment program. As part of this role, you will coordinate care for adults with complex needs and work closely with a team to ensure seamless transitions from residential to community living.

Key Responsibilities:
  • Providing after-care support to clients in residential treatment
  • Coordinating high-intensity AOD care for adults with complex needs
  • Supporting individuals' transition from residential treatment to community living
  • Triage clients based on assessment and understanding clinical risk
About You

To succeed in this role, you will need:

  • A National Criminal History Check and Working with Children Check
  • Minimum Bachelor level qualifications in a related health services discipline
  • A strong understanding of clinical risk and ability to triage clients
