The successful applicant will use a person-centred framework to develop transition plans with a key focus on finding medium to longer-term accommodation and community supports to maintain it.

Key Responsibilities
  • Facilitate the transition of participants to suitable, longer term accommodation
  • Support personal wellbeing through the provision of tailored support services to participants and provide one-on-one support
  • Making assessments to determine service eligibility
  • Work collaboratively with internal specialists and external service providers and make referrals where additional support requirements are identified to better meet client needs
  • Demonstrate cultural awareness and ability to work effectively with Aboriginal and Torres Strait Islander participants
Requirements
  • Qualification in Social Work or Community Work and/or experience engaging with individuals who are homeless or at risk of being homeless
  • Knowledge of the community services sector in South Australia with a specific emphasis on homelessness and the criminal justice sectors
  • Experience developing and maintaining support plans, and risk assessment documentation
