Case Manager PRN

This position requires a dedicated professional to manage client cases with expertise in clinical knowledge. The ideal candidate will be responsible for providing therapeutic services and coordinating continuum of care activities.

  • Provide case management and care coordination services for clients, ensuring optimal resource utilization, service delivery, and compliance with external agencies and referral source requirements.
  • Conduct psychosocial assessments and facilitate individual, family, and group sessions.
  • Coordinate treatment and discharge activities as needed.
  • Develop, implement, manage, and evaluate client care plans to ensure quality care.
  • Collect, analyze, and provide clinical data to the treatment team.
  • Correlate clinical data for the business office as required.
  • Address issues or activities relevant to the treatment team promptly.

Qualifications:

  • Bachelor's degree in Social Work, Counseling, or a related human service field is required.
  • One or more years of experience in a behavioral health setting is preferred.
  • Experience with the patient population served by the facility is preferred.
  • Experience in case management and/or utilization review is preferred.
