We are seeking a highly skilled and experienced Rehabilitation Consultant to join our team at Advanced Personnel Management. As a key member of our rehabilitation team, you will play a vital role in supporting our military personnel to achieve their health goals and return to service.
Key Responsibilities- Collaboration and Communication: Work closely with a variety of stakeholders, including employees, medical specialists, treatment providers, and allied health professionals to develop and implement effective rehabilitation plans.
- Case Management: Implement and monitor rehabilitation plans, ensuring that employees receive the necessary support and resources to achieve their health goals.
- Assessment and Review: Conduct initial assessments and perform worksite reviews to identify areas for improvement and develop targeted interventions.
- Documentation and Reporting: Maintain accurate and comprehensive records of employee progress and outcomes, and provide regular reports to stakeholders.
- Registration and Qualifications: Current unrestricted registration or membership with AHPRA as a Registered Nurse, and tertiary qualifications in Nursing.
- Experience and Skills: Proven experience in case management, clinical reasoning, and professional documentation, with a strong ability to engage across multidisciplinary teams.
- Personal Qualities: A passion for helping people rehabilitate, and a commitment to delivering high-quality services that meet the needs of our military personnel.
