Main Responsibilities

• Manage and Coordinate Care Services
• Develop and Implement Care Plans
• Liaise with Clients, Families, and Healthcare Professionals
• Foster Strong Relationships with Clients, Stakeholders, and Team Members
• Ensure Compliance with Legislation and Organisational Policies
• Monitor and Evaluate Care Services to Improve Quality and Efficiency
• Manage Home Care Packages and Client Funding

Requirements

• Experience in Case Management and Client Care Coordination
• Strong Verbal, Written, and Interpersonal Communication Skills
• Proof of Immunity and Vaccination
• Current Police Screening Certificate
• Current First Aid and CPR Certificate
• NDIS Worker Screening Check and Blue Card
• Queensland Driver's Licence
• Tertiary Qualifications in a Health-Related Field
