Home Care Nurse in Hervey Bay

We are seeking a dedicated and compassionate Clinical Registered/Enrolled Nurse to join our team in Hervey Bay, providing high-quality in-home aged care and disability support services across Australia.
Job Description:As a Clinical Registered/Enrolled Nurse, you will be responsible for assessing participants in their homes, undertaking ongoing case management, monitoring and communicating on participant wellbeing, and documenting progress using person-centred strategies.
Key Responsibilities:- Undertake assessments of participants in their homes
- Ongoing case management supporting participants in clinical and holistic needs
- Monitoring and communicating on the participant's wellbeing and reporting on progress of participants
- Documenting participant's goals, progress and outcomes using person centred, wellness, reablement and ablement supporting strategies
To be successful in this role, you will require advanced computer skills, administration skills, organisational skills, ability to multi-task and work in a fast-paced environment, strong prioritising and problem-solving skills, ability to work in a wellness model – reablement and enablement, demonstrated strengths in teamwork and working autonomously, previous experience in KPI's and working to deadlines, current CPR / First Aid Certificate, current and satisfactory Criminal Record Check, current and satisfactory NDIS Screening Card and Working With Children Check, current Australian Driver's Licence, previous experience in the home care package, community care or health/aged care sector, understanding of Home Care Package program Guidelines and Compliance or previous community nursing experience, minimum Bachelor's in Nursing - Current AHPRA, Enrolled Nurse - Current AHPRA.
