About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Focus Connect. As a Support Worker, you will play a vital role in providing assistance and support to individuals in need, helping them to live their best lives.

Key Responsibilities
  • Provide emotional and physical support to individuals, promoting their independence and well-being.
  • Assist with daily living activities, such as bathing, dressing, and grooming, ensuring the individual's dignity and comfort are maintained.
  • Help individuals with meal preparation and feeding, promoting healthy eating habits and nutrition.
  • Administer medication as directed by healthcare professionals, ensuring accurate and timely administration.
  • Accompany individuals to appointments and activities, providing transportation and support as needed.
  • Maintain accurate records of services provided, ensuring confidentiality and data protection.
  • Communicate effectively with individuals, families, and healthcare professionals, promoting open and respectful relationships.
  • Monitor and report any changes in an individual's condition, ensuring timely interventions and support.
  • Provide a safe and comfortable environment for individuals, promoting their physical and emotional well-being.
Requirements
  • Current and valid Working With Children Check clearance.
  • Current and valid full Driver's Licence and access to a comprehensively insured vehicle.
  • Current and valid First Aid Certificate.
  • Cleared National Criminal History check.
  • OR current and cleared NDIS Worker Check.
  • Experience working within Aged Care or Disability Services.
  • Ability to work within a team and also autonomously.
  • Working rights in Australia.
  • Smartphone to access rostering application.
  • Compassionate and patient demeanor.
Desirable Criteria
  • Fluent in either Spanish, Chinese, Vietnamese, or Arabic.
  • MR licence and ability to drive a bus.
