Care Coordinator

We are seeking a passionate professional to join our community care team. As a skilled healthcare provider, you will deliver person-centred care to clients in the community, ensuring a safe and healthy environment.

• Assess, plan and implement client care within your scope of practice
• Consider physical, social and psychological requirements for optimal outcomes
• Create and update care plans to reflect developing needs and priorities
• Evaluate clinical quality compliance standards and make recommendations for improvement
• Collaborate with other healthcare professionals as part of a multi-disciplinary team

You will spend most of your time on the road, conducting scheduled community visits to provide high-quality patient care.

• Full registration as a nurse with AHPRA and unlimited work entitlements
• General nursing experience post-registration, with a strong focus on community health
• Excellent communication and clinical problem-solving skills, with the ability to work autonomously
• A current Australian Driver's Licence and a reliable, insured vehicle
• Strong organisational skills, with the ability to manage competing priorities and deadlines
