CDM Nurse

About the Role:

We are seeking a highly skilled and experienced Enrolled Nurse to join our team as a Chronic Disease Management (CDM) Nurse. As a CDM Nurse, you will play a crucial role in supporting patients with chronic conditions to manage their health and prevent hospital readmissions.

Key Responsibilities:

• Prepare and implement Care Plans and Team Care Arrangements to support patients with chronic conditions
• Conduct health assessments and provide coaching to patients on health behaviour change principles
• Work closely with General Practitioners (GPs) to promote preventative healthcare and support patients in accessing ongoing care from a multi-disciplinary team
• Manage administrative tasks related to CDM appointments and ensure patient care plans are reviewed and updated regularly

Requirements:

• AHPRA registration as an Enrolled or Registered Nurse
• Proven experience in Chronic Disease Management and health assessments
• Strong organisational and time management skills, with excellent communication and customer service skills
