About the Role
  • You will conduct thorough clinical assessments and develop tailored care plans to meet the unique needs of each client.
  • Additionally, you will oversee the coordination of an allocated case load of client services and provide support to care staff as needed.
  • Direct nursing care will also be provided when required, ensuring seamless service delivery.
  • By identifying and managing clinical risks proactively, you will contribute to maintaining high-quality care standards.
  • Furthermore, you will offer training and support to team members, promoting a culture of continuous learning and growth.
  • Maintaining compliance with aged care legislation and quality frameworks is essential for this role.
Your Key Responsibilities
  • Client-Focused Care: Conduct in-home assessments and prepare individualised care plans that cater to the specific needs of each client.
  • Provide clinical education and support to clients and their families, empowering them to take an active role in their healthcare.
  • Monitor care quality closely and implement nursing interventions as necessary to ensure optimal outcomes.
  • Respond promptly to service enquiries and referrals, providing excellent customer service at all times.
  • Build strong relationships with external providers, referrers, and community stakeholders to foster a collaborative network of support.
What We're Looking For
  • AHPRA Registration as a Registered Nurse is mandatory for this role.
  • Previous experience in leadership and coordination roles within aged or community care settings is highly desirable.
  • Strong knowledge of aged care legislation, WHS, and quality standards is advantageous but not essential.
  • Excellent interpersonal, problem-solving, and documentation skills are vital for success in this position.
  • Proficiency in using clinical and scheduling software such as VisualCare or similar is also required.
