Support Coordinator Specialist

We are seeking a highly skilled Support Coordinator to join our team at nib. As a Support Coordinator, you will play a crucial role in assisting National Disability Insurance Scheme (NDIS) participants in coordinating and implementing all supports outlined in their funded plans.
Key Responsibilities- Strengthen and enhance the participant's ability to coordinate supports and ensure the delivery of their plan within the allocated budget.
- Ensure mainstream services meet their obligations to the participant.
- Build the participant's capacity to achieve greater independence and self-direct their services and supports.
- Support the participant in managing crises and building resilience.
- Provide reports on outcomes and success indicators to the participant and the NDIA.
- Develop and implement solutions to enable participants to achieve their goals and enhance their inclusion in the community.
- A competitive salary of $80,000 - $100,000 per annum, depending on experience.
