Make a Meaningful Difference as a Support Coordinator

We're seeking an exceptional individual to engage with people with disabilities and their families in creating personalized service agreements and support arrangements.

This role involves:

  • Engaging participants and their families in developing meaningful service plans
  • Empowering individuals with choice, control, and positive outcomes
  • Collaborating with teams, clients, and stakeholders to achieve desired results

The ideal candidate will possess strong communication and problem-solving skills, be able to work independently with excellent time management skills, and hold relevant qualifications such as Cert IV in Disability or Social Work.

Key Responsibilities:

  1. Develop and implement personalized service agreements and support plans for NDIS participants
  2. Promote autonomy, dignity, and well-being for people with disabilities
  3. Work collaboratively with colleagues, clients, and stakeholders to achieve desired outcomes

Requirements:

  • Relevant qualifications (Cert IV in Disability, Social Work, Community Services)
  • Strong communication and problem-solving skills
  • Excellent time management skills
