Job Title: Aged & Disability Support - Care Team Member

Care Together Pty Ltd is seeking a compassionate and dedicated Care Team Member to join our team. As a Care Team Member, you will play a vital role in providing personalized care and support to individuals within our community.

Key Responsibilities:
  • Provide Personalized Support: Offer emotional support, assist with daily activities, and promote independence and well-being in accordance with individual care plans.
  • Assist with Daily Tasks: Provide companionship, assist with meal preparation, transport, and community access, ensuring individuals' needs are met.
  • Foster a Supportive Environment: Create a safe and empowering environment that promotes independence, encourages individuals to reach their full potential, and establishes positive relationships with individuals, families, and caregivers.
  • Monitor Progress: Document individual progress, behaviors, and any changes in their well-being, maintaining accurate records.
  • Maintain a Safe Environment: Adhere to health and safety regulations, ensuring a clean and safe environment for individuals and staff.
Requirements:
  • Previous Experience: Previous experience in a similar support worker role is preferred but not essential; training will be provided.
  • Compassionate Nature: Demonstrate a compassionate and empathetic nature with a genuine desire to support others.
  • Excellent Communication: Possess excellent communication and interpersonal skills, able to work independently and as part of a team.
  • Flexibility: Be flexible to work various shifts, including no week nights or weekends.
  • Valid Driver's License: Hold a valid driver's license and have access to a reliable vehicle.
