Key Responsibilities
  • Assist participants in understanding and implementing personalized plans tailored to their unique needs and aspirations.
  • Develop and coordinate access to various services, including therapy, health care, education, and community participation, ensuring seamless navigation for our clients.
  • BUILD STRONG RELATIONSHIPS with service providers and stakeholders, facilitating effective communication and collaboration.
  • Support participants in resolving challenges or barriers in accessing services, promoting self-advocacy and empowerment.
Qualifications and Requirements
  • Proven experience as a Support Coordinator or in a related field, with a strong knowledge of disability frameworks and principles.
  • A relevant qualification in Disability, Social Work, or a related field, complemented by excellent communication, organizational, and problem-solving skills.
  • Empathy, patience, and a passion for making a positive impact in the lives of individuals with disabilities.
